We are seeking a highly skilled and compassionate Case Worker to join our Centre for Women's, Children's and Family Health in Campbelltown. As a key member of our team, you will provide effective, flexible, and holistic case management support to women and children who have experienced domestic and sexual violence.
Key Responsibilities- Work with clients to undertake comprehensive assessments to develop, implement, monitor, and review their client plan and outcomes.
- Manage and coordinate brokerage to ensure clients receive the necessary support and resources.
- Engage clients using appropriate assessment and risk minimisation processes.
- Coordinate with other service providers, including government departments and agencies, to ensure a comprehensive and coordinated response to client needs.
