Job Title:

Clinical Care Coordinator

About the Role:

The Clinical Care Coordinator plays a vital role in delivering high-quality evidence-based clinical care as part of a multidisciplinary team to achieve better health outcomes for adults aged 18 years and over who experience mental health concerns and their families and carers.

  • Conduct intake and assessments, including risk assessments, care treatment planning, care coordination with internal & external services, and provide brief psychological intervention.

Key Responsibilities:

- Conduct thorough assessments to identify client needs and develop personalized care plans.

- Coordinate care with internal and external services to ensure seamless delivery of support.

- Provide education and support to clients and their families on mental health and wellbeing strategies.

- Collaborate with multidisciplinary teams to achieve positive health outcomes.

Requirements:

  • Must have one of the following: Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist OR AASW registration/eligibility as a Social Worker (would suit graduate Social Worker wanting to build skills in the mental health field).
  • Must have flexibility to work afternoons & evenings, weekends and public holidays.
  • Knowledge & understanding of the theory underpinning evidence-based interventions & clinical practice as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use.
  • Highly developed communication (including written) and interpersonal skills.
  • The ability to engage with people, advocate on their behalf and establish and maintain effective relationships with staff, clients, families & friends and stakeholders.
  • Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
  • Must be eligible to apply/currently hold a WWCC.
  • Must hold/be willing to apply for NDIS worker screening clearance.
