Job Description

We are seeking an experienced Case Manager to join our team in the Cardinia/Gippsland region. The successful candidate will be responsible for overseeing the day-to-day care of nominated clients, providing direct care and support where required, and ensuring all care is delivered to the highest standard by skilled, caring staff.

As a key member of our team, you will assist with client growth and staff recruitment, supporting the Senior Care Manager to run the region smoothly and efficiently. You will also have the opportunity to build our reputation as the provider of choice in the community.

Key Responsibilities:

  • Oversee the day-to-day care for nominated clients
  • Provide direct care and support where required
  • Ensure all care is delivered to the highest standard by skilled, caring staff
  • Assist with client growth and staff recruitment
  • Support the Senior Care Manager to run the region smoothly and efficiently
Required Skills and Qualifications

To be successful in this role, you will need to have experience in client coordination and aged care, a proven ability to work collaboratively within a team, and commitment to meeting the Aged Care Standards and legislative requirements. You will also require relevant qualifications such as EEN, Cert IV in Community Service, Aged Care or Individual Support, or equivalent.

Additionally, you will need to hold current First Aid certification and National Police Clearance, as well as a current Driver's Licence and a fully maintained, registered, and insured vehicle. For Nurses, AHPRA registration is also required.
