Job Opportunity

The NDIS Support Partner plays a crucial role in ensuring participants receive professional service and support to meet their care needs. This is a highly skilled position that requires dedication and commitment to delivering exceptional outcomes.

Key responsibilities include:

  • Managing an assigned case load of NDIS participants, ensuring their care and services are identified and delivered to meet their care needs, NDIS goals, and preferences
  • Participant reviews and home visits to foster strong relationships and build trust
  • Ongoing assessments and support plans' reviews to ensure seamless delivery of services
  • Maintaining accurate and up-to-date participant-related documentation
  • Collaborating with internal and external stakeholders to provide best-in-class service and achieve outstanding results
  • Providing operational support to drive business growth and success

Requirements

We are seeking a passionate and driven individual with a strong background in NDIS compliance requirements and experience working in a coordination or similar role. Essential skills include:

  • Excellent communication and interpersonal skills to effectively engage with participants, families, and stakeholders
  • Strong analytical and problem-solving skills to navigate complex scenarios and deliver innovative solutions
  • Ability to work collaboratively as part of a dynamic team and contribute to a positive and supportive work environment
