Supporting Families in Crisis

The Family Housing Support Outreach Case Manager plays a pivotal role in supporting families who are homeless or at risk of homelessness by providing them with long-term, affordable housing solutions and assistance.
This position requires effective coordination with crisis services, transitional housing management properties, and local partners to improve housing stability and overall wellbeing for families in designated areas.
Key Responsibilities- Assist families in securing stable accommodation after exiting crisis housing or at referral.
- Provide case management to address needs and establish ongoing support referrals.
- Deliver rapid response to immediate housing and support needs; ensure property delivery.
- Raise awareness of available services and supports; prevent tenancy failure.
- Educate on rights and responsibilities under the Residential Tenancies Act.
- Promote wellbeing, self-efficacy, and timely access to assistance.
- Ensure services align with the National Homelessness Stream Model of Care.
- Tertiary qualifications in Social Work or a related community service field.
- Demonstrated knowledge of homelessness, its context, legislation, models, and practice.
- Experience working with culturally diverse communities.
- Strong assessment, case management, and collaboration skills.
