Your Responsibilities:

In this role, you will leverage your expertise to manage the casework of our Community Aged Care clients effectively. Your proactive approach and ownership of your caseload will be crucial for success.

  • Conduct assessments, coordinate, and review client care requirements within their allocated budget.
  • Deliver tailored, flexible services that meet individual needs while providing advocacy and referral support.
  • Demonstrate a genuine passion for care, aiming to maximize service delivery while ensuring sustainable case management aligned with organizational goals.

Ideal Candidate:

  • Proven experience in case management, particularly with Home Care Packages in the Aged Care sector or relevant skills from other fields.
  • Strong planning, negotiation, and influencing abilities.
  • Experience managing compliance with government program requirements, ensuring adherence to regulatory standards and organizational policies.
  • High level of computer proficiency and adaptability to new software systems.
  • Possession of a registered and insured vehicle along with a current unrestricted driver's license.
