Senior Mental Health Care Coordination Specialist

Care Coordinator Specialist This is an exciting opportunity for a skilled care coordinator to join our dynamic multidisciplinary team, providing comprehensive care coordination for older adults with complex mental health issues.
To be successful in this role, you will have strong organizational skills, the ability to work effectively across multiple sites, and a passion for delivering high-quality patient-centered care. You will also possess excellent communication skills, both verbal and written, and be able to educate and guide internal and external partners.
The key requirements for this position include:
- A Registered Nurse with at least 5 years of full-time postgraduate experience in mental health.
- Experience working with consumers and carers with complex mental health needs, including assessment, planning, and evaluation of interventions.
- Extensive experience in conducting mental health assessments and developing evidence-based, consumer-focused care plans.
