Clinical Care Coordinator

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Estia Health. As a Clinical Care Coordinator, you will play a vital role in delivering exceptional care and support to our residents.
Key Responsibilities- Drive a continuous improvement culture to enhance the quality of care and services
- Act as a clinical resource for staff, providing education and guidance on clinical practices
- Focus on clinical service delivery, compliance, and audit processes to ensure high standards are met
- Care planning in line with new standards to ensure resident-centered care
- Take pride in your clinical documentation management skills to maintain accurate and up-to-date records
- Responsible for infection prevention strategies to ensure a safe environment for residents and staff
- Provide training and coaching to staff on infection practices and documentation
- A real desire to make a difference in people's lives and provide exceptional care
- Leadership experience in clinical care, with a focus on aged care
- A nursing degree and current AHPRA registration
- Solid documentation skills and knowledge of ANAAC
