Empower Complex Care Needs

We are seeking a highly skilled and empathetic Senior Support Coordinator to join our team. As a key member of our Allied Health Services, you will be responsible for empowering individuals with complex care needs by providing them with personalized support and connecting them with relevant services.
Your Key Responsibilities:- Conduct comprehensive assessments to identify individualized needs and develop tailored care plans.
- Carefully coordinate services to ensure seamless transitions and optimal outcomes.
- Foster strong relationships with service providers to advocate for customer needs and preferences.
- Promote health literacy and self-management among customers to enhance their overall wellbeing.
To succeed in this role, you will possess a minimum of 2 years' experience in case management, alongside a solid understanding of services catering to younger people with disabilities. Your expertise in psychosocial assessments, care planning, and service coordination will be invaluable. A tertiary qualification in Social Work, Community Services, or equivalent is also essential.
