Dynamic Case Manager Opportunity

We are seeking a highly skilled and compassionate case manager to join our team. As a dynamic case manager, you will have the opportunity to work with individuals who have experienced brain injuries.

About the Role:

  • This is a challenging yet rewarding role that requires strong social work and case management skills.
  • You will be responsible for providing evidence-based clinical services and rehab case management to clients admitted to the Brain Injury Unit (BIU).
  • Counsel clients and their family/carers to deal with the emotional impact of injury, illness, and disability.
  • Develop and implement case plans, liaise with multidisciplinary teams, and advocate for clients to maximize access to community services.
  • Discharge planning is an essential element of this role, requiring close collaboration with clients, families, and healthcare professionals.
  • Supervise students, provide education and teaching, and contribute to the ongoing development of social work and case management services.

Your Qualifications:

  • Hold tertiary qualifications in Social Work and be eligible for membership of the Australian Association of Social Workers, demonstrating a commitment to the AASW Code of Ethics.
  • Have extensive experience in social work, clinical skills, and expertise in assessment, counseling, advocacy, referral, and discharge planning in a health/rehabilitation setting.
  • Be familiar with the National Disability Insurance Scheme (NDIS) and its implications for discharge planning.
  • Have a minimum of 3 years' experience working in a health setting with adults with physical or cognitive disabilities.
  • Demonstrate excellent communication skills, negotiation, conflict resolution, time management, and ability to set priorities and complete tasks.
  • Be committed to participation in supervision and professional development, with computer skills and demonstrated competency.
  • Understand quality improvement processes and their implementation, as well as knowledge of Case Management Standards.
  • Hold a C Class drivers' licence.
