Lead Mental Health Care Navigator

We are seeking a highly skilled and experienced professional to fill the role of Clinical Nurse Consultant - Navigator (Mental Health).
This position plays a key lead in coordinating patient flow across inpatient units and linking directly to broader patient flow for the hospital through the Integration Operations Centre.
The successful candidate will have:
- Demonstrated specialist-level knowledge and leadership skills within mental health acute care settings.
- High-level interpersonal and communication skills, including mediation and conflict resolution.
- Effective organisational skills and the ability to problem-solve and apply principles of clinical risk management, quality improvement, and evidence-based practice.
Main Responsibilities:
- Coordinate patient flow across inpatient units ensuring seamless transitions and optimal outcomes.
- Develop and implement effective care plans that cater to individual patient needs.
- Collaborate with multidisciplinary teams to provide versatile, flexible services that meet the complex demands of mental health care.
