Job Title: NDIS Support Worker

We are seeking an experienced NDIS Support Worker to join our team in Perth, WA. As an NDIS Support Worker, you will provide support to participants with identified goals as per their NDIS plan. This role involves working closely with participants to break down their goals into achievable steps and supporting them to achieve positive outcomes.

Key Responsibilities:
  • Provide high-quality support to participants who require assistance with daily living tasks, personal care, and social activities.
  • Develop and implement individualized support plans that meet the unique needs and goals of each participant.
  • Work collaboratively with internal and external stakeholders to ensure seamless delivery of services.
  • Maintain accurate and up-to-date documentation, including progress reports and service agreements.
  • Contribute to a culture of teamwork and respect, promoting a positive work environment.
Requirements:
  • A minimum of 6 months' experience in NDIS Services or a related field.
  • Cert III in Mental Health, Disability Service, Community Services, with a view of working towards a Cert IV.
  • Seizure management training.
  • Demonstrated ability to provide individualized, goal-oriented care and support.
  • Understanding of personal and emotional recovery principles and capacity building.
