About the Role

We are seeking a dedicated and compassionate Clinical Care Coordinator to join our team at Germanus Kent House in Broome. As a key member of our clinical team, you will be responsible for coordinating and leading the delivery of person-centered care to our residents and clients in the community.

Your Responsibilities

  • Supervise and direct the delivery of client-focused care delivered by staff">
  • Assess the total health needs of clients and document results thoroughly">
  • Develop individual care plans in consult with clients and significant others">
  • Comply with all relevant legislation and policies">
  • Uphold the organization's values and principles in all areas of service delivery">

About You

">

To be successful in this role, you will hold a valid AHPRA registration and possess great interpersonal and communication skills. You will be detail-oriented and able to work well in both a team environment and independently. Experience in Aged Care is ideal, but not necessary.
