As a skilled and compassionate clinical professional, you will play a vital role in supporting the health and wellbeing of our clients. You will be responsible for delivering high-quality care services to individuals with complex needs, working collaboratively with multidisciplinary teams to ensure seamless care delivery.
About the Position- Develop and maintain strong relationships with clients, carers, and families, providing emotional support and guidance as needed.
- Conduct comprehensive assessments to identify client requirements, develop individualised care plans, and ensure compliance with relevant standards and regulations.
- Deliver a range of clinical services, including medication management, wound care, chronic disease management, and palliative care, always prioritising the client's dignity and comfort.
- Act as a liaison between referral sources, families, and the care team, ensuring effective communication and collaboration to achieve optimal outcomes.
- Provide mentorship and support to non-clinical team members, promoting a culture of excellence and continuous improvement.
- Supervise and guide non-clinical team members to maintain high standards of care and service delivery.
- Participate in on-call rosters when required, demonstrating flexibility and commitment to meeting client needs.
- Collaborate with the care team to provide comprehensive care services, ensuring continuity and coordination of care.
- Qualified Registered Nurse with current registration, possessing excellent clinical skills and knowledge.
- Strong organisational and problem-solving abilities, with a proactive approach to addressing challenges and improving processes.
- Excellent written and verbal communication skills, with the ability to build rapport with clients, carers, and colleagues alike.
- Minimum 2 years' experience in a care management/coordination role within aged care or community care, with a proven track record of delivering high-quality care services.
