About the Role

We are seeking a full-time administrator to provide support for our care management and package management services, including care coordination, to home care package recipients. The successful applicant will champion a consumer-directed approach and provide ongoing support to consumers.

Key Responsibilities:

  • Onboard new clients in our CRM system
  • Support care coordinators in onboarding to our care coordination program
  • Capture and enter data for care budgets and care plans
  • Provide administrative support and document development to improve resources for care coordinators
  • Liaise with clients and/or their representatives on aspects of their home care package
  • Coordinated and perform periodic client service reviews; requests for upgrades and supplements; leave and other related activities
  • Maintain and update care management systems relating to clients and their representatives, stakeholders, service providers and government agencies
  • Acknowledge and action requests for consumers from care coordinators
  • Schedule care plan meetings with consumers, their representatives and coordinators as required

Requirements:

  • Current Federal Police Clearance (not more than 12 months old)
  • Evidence of COVID Vaccination (industry requirement)
  • Evidence of Australian work rights (Passport, Birth Certificate, etc)
