About the Role

As a Community Outreach Coordinator, you will work closely with families to develop and implement personalized case plans that address their unique needs. This outreach-based role requires strong communication and interpersonal skills, as well as the ability to work effectively in a performance-driven environment.

Key Responsibilities:
  • Family Support: Provide high-quality support to families, working closely with them to identify and address their challenges.
  • Case Planning: Develop and implement effective case plans that align with our organization's values and vision.
  • Outreach and Engagement: Engage clients through assertive outreach approaches and facilitate client-led assessments and care planning.
  • Collaboration: Collaborate with internal and external stakeholders to provide holistic care for families.
Requirements
  • A current degree in Social Work, Human Services, or a related field, combined with relevant experience.
  • The ability to work in a fast-paced environment and meet targets.
  • An understanding of behavior change interventions and factors contributing to vulnerability.
  • Experience in case management for high-risk clients and coordination of multiple services.
