Job Description

We are seeking a skilled and compassionate Home Care Partner to join our team. As a Home Care Partner, you will work closely with customers to enhance their quality of life, enabling them to remain at home while meeting their care needs.

You will provide case management for customers, fostering independence and promoting well-being - case management experience and qualifications are essential.

Our ideal candidate will be located in the Eurobodalla/Far South Coast region or the ACT, with a preference for a hybrid position working from the nearest office at least 3 days a week and 2 days working from home.

Key responsibilities include:

  • Receiving and assessing referrals, screening customers for eligibility, and ensuring safe home access for staff.
  • Contacting newly referred customers to discuss expectations, goals, and available services.
  • Conducting comprehensive assessments to identify customer needs and goals.
  • Collaborating with customers to achieve goals by coordinating and suggesting appropriate services.
  • Conducting regular customer reviews via telephone and reassessing care plans as needed.
  • Maintaining accurate customer care plans and ensuring compliance with funding bodies and leadership.
