Nurse Care Specialist Role

About the Job
  • You will work closely with clients, their carers, General Practitioners, and other relevant service providers to develop personalized care plans that meet the unique needs of each client.
  • You will provide high-quality nursing care, promote health education, and support clients and their families in managing chronic conditions and maintaining independence.

Required Skills and Qualifications
  • A registered nurse with current registration and a valid practicing certificate.
  • Experience in hospital-based or community-based nursing, preferably in a similar role.
  • Demonstrated expertise in developing and implementing individualized care plans that cater to diverse client needs.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers, and healthcare professionals.
