Nurse Care Specialist Role
As a skilled and dedicated nurse care specialist, you will have the opportunity to make a meaningful difference in the lives of people in your community. Our Hospital in the Home service is an integral part of the total healthcare delivery system, providing nursing care to individuals in their own homes.
About the Job- You will work closely with clients, their carers, General Practitioners, and other relevant service providers to develop personalized care plans that meet the unique needs of each client.
- You will provide high-quality nursing care, promote health education, and support clients and their families in managing chronic conditions and maintaining independence.
- You will be part of a dynamic team that values collaboration, flexibility, and continuous learning, offering opportunities for professional growth and development.
- A registered nurse with current registration and a valid practicing certificate.
- Experience in hospital-based or community-based nursing, preferably in a similar role.
- Demonstrated expertise in developing and implementing individualized care plans that cater to diverse client needs.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers, and healthcare professionals.
