The Role:
- Conduct thorough assessments and create personalized care plans in collaboration with consumers and/or their representatives, aligning with individual needs and professional guidelines.
- Maintain effective communication with medical professionals, allied health experts, pharmacies, service providers, and equipment suppliers to deliver comprehensive care.
- Diligently document and report care interventions, observations, changes in needs, and consumer feedback in a clear, objective, and efficient manner.
- Coordinate staff as required on a shift-by-shift basis.
- Assist in reviewing and disseminating clinical policies, procedures, and assessment tools under the direction of the Clinical Nurse Manager.
- Provide expert guidance and education to personal care staff to ensure high-quality care.
The ideal candidate will have:
- Current Registration as a Registered Nurse with AHPRA
- Experience in Aged Care, preferably in a residential setting
- Able to Work Various Shifts, Including Days, Evenings, and Nights
- Able to Commit to Permanent Shifts - 10 Shifts Per Fortnight
- Sound Understanding of Aged Care Accreditation Standards
- Current National Police Clearance and NDIS Screening
- Up-to-Date Influenza and COVID Vaccinations
