Job Title: Home Care Package Case Manager – Aged Care

We are seeking an experienced Case Manager to join our team at Superior People Recruitment. The successful candidate will be responsible for managing and supporting up to 60 clients receiving Home Care Packages (HCP) in accordance with program guidelines.

Key Responsibilities:
  • Manage client care plans, including initial assessments, service coordination, and regular reviews.
  • Provide ongoing case management to clients, ensuring their care needs are met through individualised plans.
  • Ensure accurate completion of documentation and compliance with Home Care Standards and Aged Care Commission regulations.
  • Maintain up-to-date client records in the management system (Etools/Carelink+).
  • Liaise with family members, carers, GPs, and other stakeholders to ensure seamless care provision.
  • Handle client complaints and grievances, collaborating with internal teams to resolve issues.
  • Assist with new client onboarding, recruitment, staff induction, and professional development.
Requirements:
  • Strong knowledge of Aged Care guidelines and Consumer Directed Care.
  • Experience with case management and using client management systems.
  • Excellent communication and organisational skills.
