Clinical Hospital Pharmacist

We are seeking a skilled and dedicated Clinical Hospital Pharmacist to join our dynamic team. As a key member of our healthcare team, you will be responsible for providing high-quality clinical pharmacy services and supporting medication management across a wide range of specialties.

  • This is a permanent part-time opportunity with a Saturday shift from 8:00am to 1:00pm, working a total of 10 hours per fortnight.
  • No prior hospital pharmacy experience is required as comprehensive training will be provided.
About the Role

As a Clinical Hospital Pharmacist, you will work closely with a multidisciplinary team to optimise patient outcomes. Your day-to-day responsibilities will include:

  • Delivering clinical reviews, medication history taking, discharge counselling, and collaborating with healthcare professionals to ensure seamless patient care.
  • Rotating through wards and dispensary, ensuring efficient and effective dispensing and distribution of medication.
  • Conducting medication reconciliations, preparing discharge summaries, and offering medication counselling and education to patients.
  • Collaborating with healthcare teams to provide advice on patient care and contributing to training and educational sessions for staff members.
Requirements

To be successful in this role, you will need to possess:

  • A current Pharmacist registration with AHPRA.
  • Relevant pharmacy experience in Australia or New Zealand.
  • Excellent communication skills to effectively liaise with a wide range of healthcare professionals.
  • A passion for building on your clinical skills and knowledge.
