About This Role

We are seeking a dedicated professional to support young people transitioning from care.

This role involves creating and implementing quality plans tailored to each individual's needs, ensuring their safety and well-being. The ideal candidate will have excellent communication and interpersonal skills, with the ability to build trust and rapport with clients.

The key responsibilities of this position include:

  • Developing and maintaining strong relationships with clients, their families, and other relevant stakeholders.
  • Collaborating with colleagues to share knowledge and best practices in casework.
  • Providing advocacy and support to help clients access necessary resources and services.
  • Maintaining accurate records and reporting on client progress.
Requirements

To be successful in this role, you will need:

  • A Bachelor Degree in social work, social science, community welfare, or a related field.
  • A valid Australian Driver's Licence.
  • Minimum 2 years' experience in a related field.
  • A paid working with children clearance number.
