Highly Skilled Care Professional Wanted

We are seeking a highly skilled professional to provide exceptional care and support services to consumers and their carers, ensuring optimal health outcomes aligned with our policies and procedures.
The ideal candidate will work independently and collaboratively within a multidisciplinary team, prioritizing tasks and managing stressful situations. They will demonstrate outstanding communication skills, both written and verbal, and possess a proven ability to work autonomously and as part of a team.
The role involves delivering a range of social work interventions, including home visiting models, and collaborating with various stakeholders, such as DCJ, NGOs, Youth Health PARVAN, Child and Family Health, Paediatric, Mental health and allied health teams.
Key Responsibilities- Provide high-quality social work services to consumers and their carers to ensure optimal health outcomes.
- Participate in professional development, clinical supervision, quality activities and other relevant departmental and administrative duties.
- Work independently and collaboratively within a multidisciplinary team.
- Demonstrate exceptional communication skills, both written and verbal.
- A Social Work degree that provides eligibility for membership of the Australian Association of Social Workers (AASW).
- Demonstrated experience in a health-related field and ability to deliver a range of social work interventions.
- Proven ability to work independently and collaboratively within a multidisciplinary team.
- Excellent communication skills, both written and verbal.
- Australian C-Class driver's license (including P1 or P2). Those who hold a C-class license (P1 or P2 included) from a state other than NSW must obtain a NSW C-class (P1 or P2 included) license within three months of appointment.
