As an ESTEP Care Coordinator, you will be responsible for managing a caseload while adhering to established protocols and procedures.
Key Responsibilities- Care Plan Development
Create and continually update person-centred care plans that cater to the unique needs of each client.
Therapeutic RelationshipsBuild trust and rapport with clients through effective communication and empathy.
Risk Assessment and ManagementIdentify and mitigate potential risks by designing personalized risk management plans.
Leadership and SupervisionProvide guidance and support to junior staff members through supervision and appraisal.
Team CollaborationParticipate in multidisciplinary team (MDT) activities, including coordinating meetings and undertaking duty responsibilities.
Safeguarding and ComplianceEnsure adherence to relevant laws and regulations, including the Mental Health Act 1983.
Client SupportMonitor physical health and promote wellbeing, as well as support carers and service users within the service.
Additionally, you will provide profession-specific interventions for the wider team as required and participate in training and development opportunities.
