Role Overview

The Community Health Nurse position at NSW Health involves delivering evidence-based care that promotes positive health outcomes under the indirect supervision of a Registered Nurse.

You will work collaboratively with clients and the healthcare team to implement, monitor, and evaluate nursing care plans.

Key Responsibilities
  • Deliver holistic, client-centred care to individuals and families in their own homes
  • Work within a multidisciplinary team to develop, implement, and evaluate nursing care strategies
  • Provide wound assessment and management, urinary catheter care, and palliative care support
