Support Coordinator Opportunity

We are seeking a highly motivated and enthusiastic Support Coordinator to join our dedicated team. As a Support Coordinator, you will enjoy working with diverse age groups, managing a varied caseload, and providing holistic services to participants to create meaningful outcomes.

  • Blossom Community Care is a registered NDIS provider offering a range of services including Support Coordination, Plan Management, Support Workers, Allied Health Services, and Cleaning and Gardening Services.
  • We aim to create a fully supported lifestyle by providing awareness and care to individuals and their families.

Key Responsibilities:

  • Service Delivery: Provide support to participants in achieving their goals and objectives.
  • Community Engagement and Business Development: Develop and maintain relationships with stakeholders, and identify opportunities for growth and expansion.
  • Teamwork and Development: Collaborate with colleagues to achieve shared goals, and participate in training and professional development programs.
  • Legislative and Policy Compliance: Ensure adherence to relevant laws, regulations, and policies.
  • Reporting, Documentation, and Administration: Maintain accurate records, prepare reports, and perform administrative tasks as required.

Qualifications and Experience:

  • Minimum Diploma in Disability, Community Services, or related field.
  • Minimum 2 years' experience working as an NDIS Support Coordinator.
  • Current Victorian Driver's license.
  • Proven ability to work under broad directions, fit to operate screen-based equipment, and possess excellent communication and interpersonal skills.
