Care Coordinator for Aboriginal Health Services

Our organization is seeking a highly skilled and dedicated Aboriginal Health Practitioner or Registered Nurse to join our team.
Job DescriptionThe successful applicant will be responsible for providing high-quality, comprehensive, and culturally appropriate primary health care to Aboriginal and/or Torres Strait Islander people living in the Darwin area. This includes delivering patient-centered care, conducting health assessments, and developing individualized care plans that cater to the unique needs of our clients.
We are looking for someone who is passionate about working with diverse populations and has a strong understanding of cultural safety principles. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively within a multi-disciplinary team environment.
Key Requirements- Bachelor of Nursing and/or Certificate IV in Aboriginal & Torres Strait Islander Primary Health Care (Practice).
- Current registration as an AHP/ Nurse with AHPRA.
- Strong understanding of and/or experience in providing primary health care, utilizing standard treatment manuals.
- Demonstrated awareness of and sensitivity to Aboriginal culture and history, with knowledge of issues impacting on Aboriginal and Torres Strait Islander peoples today.
- Excellent written, verbal, and interpersonal skills, including the ability to communicate sensitively in a cross-cultural environment.
- Able to work independently and as part of a team, maintaining professionalism, confidentiality, and discretion.
- Strong organizational skills, with the ability to plan, manage, and prioritize tasks efficiently.
- Proficient in using Microsoft Office applications and clinical practice software.
