Senior Homecare Coordinator

As a Senior Personal Carer, you will conduct initial client intake and screening assessments, developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.
Key Responsibilities:- Conduct thorough initial screening assessments to provide comprehensive support for personal self-care, general house cleaning, social support, respite, transport, and shopping assistance.
- Develop tailored care plans that cater to the unique requirements of each client.
- Manage client information in our CRM & CMS system, ensuring all details and case notes are up-to-date and easily accessible.
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
- Certificate III Aged Care, Individual Support or an equivalent qualification is essential for this role.
- A current Australian Driver's Licence (Green Provisional minimum) is necessary for transporting clients.
- A roadworthy and insured vehicle (Third Party Property minimum) is required for work-related transportation.
- Proficiency with technology, including smartphones, tablets, laptops, etc., is expected.
- The ability to satisfy relevant probity checks required by legislation or policies is crucial.
