We are seeking a highly skilled and experienced Home Care Package Coordinator to join our team at Yorke and Northern Local Health Network. As a key member of our Community Health team, you will play a vital role in managing Home Care Package services under our Consumer Directed Care model.
Key Responsibilities- Coordinate Home Care Package (Aged) service agreements to ensure targets are met and clients receive culturally appropriate and empathetic support.
- Monitor budgets and produce reports to promote Country Health Connect as a leading provider.
- Supervise teams, including program planning, development, implementation, evaluation, and budget management.
- Work with government, non-government, and community groups to meet service obligations under formal contracts and agreements.
- Develop and implement strategies to promote client and carer needs related to age-related disorders, relevant legislation, industrial agreements, standards, and ethics.
- Proven experience in supervising teams and managing client services.
- Comprehensive understanding of client and carer needs related to age-related disorders, relevant legislation, industrial agreements, standards, and ethics.
- Knowledge of Primary Health Care principles, Evidence-Based Practice, and Work Health and Safety regulations.
- Familiarity with Quality Improvement Principles and their application in health services.
To excel in this role, you should have:
- Proven ability to work with clients across a range of disability levels and manage tasks independently while meeting deadlines.
- Strong communication skills for effective consultation, negotiation, and advocacy at various organizational levels.
- Experience in supervision, program planning, budgeting, and reporting within a multidisciplinary health-related team.
