About the Role
We are currently seeking a part-time Care Coordinator to join our team in Gladstone. The successful candidate will provide case management support to individuals who are homeless, at risk of homelessness or housed but vulnerable.
What You Will Do
- Work closely with participants to develop and review individualised care plans that meet their unique needs.
- Provide practical and emotional support, including crisis budgeting, information, advice and community referrals.
- Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning where necessary.
- Prioritise and deliver support that is proactive, persistent, reliable, practical and comprehensive, while respecting the dignity and privacy of each individual.
Mandatory Requirements
- Certificate IV qualification in Community Services or similar field.
- 2 years' relevant experience working with vulnerable populations.
