About the RoleIn this role, you will work closely with individuals to understand their unique situation and needs, providing informed referrals to relevant services and supports. Your expertise will be essential in helping individuals navigate the complex mental health and social emotional well-being system, ensuring they receive the best possible care and support.
You will also be responsible for maintaining ongoing communication with service users and providers, ensuring a seamless and supportive experience. Key Responsibilities- Provide expert guidance and support to individuals navigating their mental health and social emotional well-being journey
- Conduct comprehensive assessments to understand individual needs and circumstances
- Develop and implement personalized plans to address individual needs and goals
- Maintain ongoing communication with service users and providers to ensure seamless support
To be successful in this role, you will need:
- A degree in Social Work, Nursing, or other Allied Health Professions
- NDISWS and WWCC qualifications
- A strong understanding of mental health and social emotional well-being principles and practices
