We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team at Salt Medical Recruitment Agency.
Key Responsibilities:
- Provide high-quality patient care in a Primary Health setting, utilizing expertise in First On Call/emergency response and Chronic disease management.
- Collaborate with multidisciplinary teams to deliver comprehensive care and support.
- Develop and implement effective care plans, ensuring patient-centered and evidence-based practice.
- Contribute to the development and maintenance of policies and procedures, ensuring compliance with regulatory requirements.
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient outcomes.
Requirements:
- Minimum of 5 years' experience in Primary Health, First On Call/emergency response, and Chronic disease management.
- Current certifications in ALS, PALS, MEC, and Triage.
- Ability to work rotating 8 and 12 hour shifts, including night shifts and weekends.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to adapt to changing priorities and deadlines, with a focus on delivering high-quality results.
