Job Description:
Aged care assessors conduct home visits to identify the complex needs of clients and refer them to appropriate aged care services under the Aged Care Act. Daily responsibilities include collaborating with clients, families, and multidisciplinary teams, conducting accurate assessments using the Integrated Assessment Tool (IAT), reviewing support plans, working autonomously, maintaining knowledge of funded and non-funded aged care services, and contributing to continuous improvement.

Required Skills and Qualifications:
Health-related tertiary qualification, current unrestricted registration with AHPRA, Australian driver's license, and access to a registered, insured vehicle. Minimum one year of experience delivering aged care services in Australia or overseas, including to people living with dementia, and willingness to apply for a Police Check.
