Mental Health and Community Living Specialist

The Pathways to Community Living Initiative (PCLI) is a specialist service based at Citigate Mental Health Centre in North Gosford.
- Expert care and support are provided for adults with complex mental illness, focusing on recovery-oriented, person-led care.
This role involves working collaboratively with multidisciplinary healthcare teams across Central Coast Local Health District.
- Key Responsibilities:
- Develop partnerships for individuals with severe and persistent mental illness, complex care needs, and alcohol or other drug use.
- Work effectively in collaboration with the multidisciplinary health care team.
- Implement new models of care partnerships for individuals with severe and persistent mental illness, complex care needs, and alcohol or other drug use.
- AHPRA registration as a Registered Nurse, Psychologist, Occupational Therapist, or eligibility for membership with AASW for Social Workers.
- Relevant post-registration qualification and experience working in the clinical area.
- Demonstrated ability to work collaboratively with multidisciplinary healthcare teams.
- Ability to provide expert care and develop partnerships for individuals with severe and persistent mental illness, complex care needs, and alcohol or other drug use.
