About the Role

We are seeking a skilled and compassionate Case Manager to join our Child, Youth and Family Services team at Anglicare NSW South, NSW West & ACT. As a key member of our team, you will play a critical role in delivering client-focused and outcome-based services to vulnerable families in our community.

Key Responsibilities
  • Advocacy and Case Planning: Work collaboratively with clients to identify their needs and develop tailored case plans that address their complex issues.
  • Trauma-Informed Care: Commit to providing trauma-informed care under the Sanctuary model, ensuring that our services are responsive to the needs of our clients.
  • Community Engagement: Foster respectful relationships with stakeholders, government departments, and the community to ensure seamless service delivery.
  • Family Support: Develop and implement case plans in collaboration with families and other service providers, ensuring that the voices of clients and families are heard in case planning and goal setting.
  • Playgroup Support: Support and engage parents and children attending our Bradfordville supported Playgroup.
Requirements
  • Qualifications: Relevant qualifications in the Human Services field (Diploma of Community Services or equivalent; Degree qualifications preferred).
  • Experience: Practical experience in working with vulnerable families and an ability to identify children and young people at risk.
  • Expertise: Demonstrated expertise in client-centred case management.
  • Knowledge: Knowledge of the Community Services sector, trauma and its impacts, crisis impacts on families, child protection issues, and legislation.
