Job Title: Client Service Coordinator

We are seeking a highly skilled and experienced Client Service Coordinator to join our team at Alzheimer's Queensland. As a key member of our team, you will play a vital role in delivering high-quality community aged care services to our clients.

Key Responsibilities:
  • Provide leadership and guidance to Community Support Assistants to ensure the delivery of high-quality services and compliance with organisational policies and procedures.
  • Ensure that services are compliant with the Aged Care Standards and other relevant legislations, and that the region is ready for an audit at all times.
  • Act as the first point of contact for client enquiries and assist with resolving incidents, compliments, and complaints in a timely manner.
  • Support staff competency assessments, training, and professional development, including monitoring and follow-up with employees.
  • Assist with the recruitment and selection, onboarding, and supervision of new personnel, as well as ongoing professional development of Community Support Assistants.
  • Perform ongoing CHSP client reviews, ensuring the appropriate MAC referral codes are in place for the client to be fully supported.
  • Manage care planning and case management of existing clients, including all relevant documentation.
  • Process staff timesheets and leave applications.
Requirements:
  • Minimum of Certificate IV (or equivalent) in Aged Care / Individual Support / Disability / Home and Community Care or in a related discipline.
  • Extensive demonstrated experience in a similar leadership role in a healthcare or disability setting.
  • Sound understanding of community Charter of Aged Care Rights and demonstrated ability to uphold those rights at all times.
  • Demonstrated excellent written and verbal communication skills, including understanding of scope of practice.
  • Good time management and problem-solving skills, with ability to work autonomously with minimal supervision and managing conflicting priorities while maintaining high attention to detail.
  • Proficiency in use of email, digital applications, and software packages, including Riskman, Microsoft Suite Package, client information systems, and mobile apps.
