About This Role
We are seeking a Home Care Professional to join our team in Morwell, supporting older adults in our community to live well at home with tailored care that meets their needs and goals.
Job Summary
The ideal candidate will be the key contact for a group of Home Care Package clients guiding them through every stage of their journey from intake to ongoing support. The role includes:
- Meeting with new clients and providing support during the onboarding process
- Completing comprehensive assessments and developing individualised care plans
- Working with families, health professionals, and service providers to deliver quality care
- Monitoring client needs, reviewing progress, and making timely adjustments when required
- Ensuring client care aligns with our values and Aged Care Standards
This is a varied and rewarding position balancing professional expertise with compassion, advocacy, and problem-solving to ensure clients feel respected, valued, and supported.
