About the Role

We are seeking Support Workers to join our team in a dynamic and rewarding role. The ideal candidate will have excellent communication skills, be empathetic and non-judgmental, and have a passion for delivering high-quality support services.

The successful applicant will be required to work collaboratively with customers, families, caregivers, and key stakeholders to develop and implement care plans that promote independence and well-being.

In this role, you will have the opportunity to make a positive impact on people's lives and contribute to creating happy, inclusive communities.


Responsibilities
  • Provide individualized support to improve independence by assisting with daily living tasks, transport, social activities, medication management, and personal care.
  • Maintain accurate and up-to-date customer records using online systems.
  • Develop and maintain professional relationships with customers, families, caregivers, and key stakeholders.
  • Contribute to the development or implementation of care plans that meet individual needs and preferences.

Requirements
  • A certificate level qualification in Disability or Community Services/individual support or equivalent relevant experience in the human services industry.
  • A valid NSW Driver's C class licence, with the ability to drive vehicles in licence class, including large cars and vans.
  • Physical fitness to undertake the demands of the role, including the capacity to lift 10 kilograms unassisted.
