What You’ll Do:

In this dynamic and rewarding role, you’ll:

- Partner with participants to design and implement personalised, strengths‑based NDIS plans.

- Connect people with the services and supports that align with their goals and needs.

- Use recovery‑oriented and person‑centred approaches to build skills and confidence.

- Coordinate service delivery, monitor progress, and adapt plans as required.

- Collaborate with a multi‑disciplinary team and contribute to service innovation.

- Maintain accurate records and ensure alignment with NDIS practice standards.

What You’ll Bring:

- Significant experience in the disability or human services sector.

- Knowledge of intervention models and ability to coordinate services.

- Specialist knowledge of NDIS and related support services.

- Strong problem‑solving and conflict‑resolution skills.

- Ability to advocate effectively for participants and promote their independence.

Your Toolkit:

- Tertiary qualification in community services, social work or equivalent and extensive community work and training (essential)

- Current DHS Working with Children Check (essential)

- Current SA driver’s licence (essential)

- Current Safe Environments for Children and Young People– Through Their Eyes Certificate (essential)
