Physiotherapist - Rehab and Restorative Care

Your Opportunity:

This role will see you delivering evidence-based community care through programs like Rehab at Home, Hospital in the Home and Restorative Care Programs. You will be joining a large and collaborative multidisciplinary team providing personalised treatments. You will be involved in a new innovative way of offering allied health services to our wonderful Clients.

Duties will include, but not limited to:

- In the community, the consumers you will be seeing in the community are referred through the following government funded models, Home Care Packages, Short Term Restorative Care, Commonwealth Home Support Programme and Private funding.

- Implement active exercise-based, people centred approaches for post-surgery orthopaedic, oncology and general medical

- Provide comprehensive assessments and clinical recommendation to support consumer activities of daily living, mobility, and transfers, falls risk and prevention, equipment requirements, pressure area care and skin integrity management, arthritic joint management, and pain management.

- Demonstrate flexibility and adaptability to the needs of Plena and
