GP Practice Nurse

An opportunity has arisen to join our friendly practice as a Practice Nurse. You will be part of a team which prides itself in providing a high standard of care for our patients.
The ideal candidate will already have all basic nursing skills, including baby immunisations, cytology and chronic disease management including a special interest in diabetes and travel immunisations.
Main duties of the job
Main Duties of the Job to include:
- Assist in and perform routine tasks related to patient care as directed by GPs
- Cytology
- Removal of Sutures
- Travel Clinic
- ECGs
- Venepuncture
- Routine immunisations/Childhood immunisations
- Requesting pathology tests, for example urine culture, swabs
- Chronic disease management, particularly Diabetes
- Following agreed clinical protocols with referral to GPs as appropriate

The Practice is based on the outskirts of Northampton Town Centre, in purpose built premises. We are high achieving, with approximately 7000 patients.
We use Systm One. You would be supported by our multi-disciplinary team.

Job Description:
Thefollowing are the core responsibilities of the Practice Nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:
a.Tosupport the Infection Prevention Control (IPC) lead and to work to the IPCrequirements by providing audits, training, support and evidence of complianceto the practice manager
b.Todevelop, implement and embed an effective care management programme for People with Learning Disabilities.
c.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice
d.Todevelop, implement and embed health promotion and wellbeing programmes with the nursing team.
e.Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing
f.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health
g.Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE
h.Asrequired, to diagnose and manage acute and chronic conditions
i.Providewound care (ulcer/Doppler, etc.) to patients
j.Toreview patient medications to enhance compliance
k.Toprovide guidance to patients in the use of over-the-counter medications regarding side effects and dosages
l.To assistwith the provision of travel medicine services
m.Tomaintain accurate clinical records in conjunction with extant legislation
n.Toensure SNOMED CT codes are used effectively
o.Toensure the appropriate maintenance and use of chronic disease registers
q.Toensure compliance with the practice chaperone policy
r.Toassist GPs with minor surgery when required
s.Toprioritise health issues and intervene appropriately
t.Tosupport the team in dealing with clinical emergencies
u.Torecognise, assess and refer patients presenting with mental health needs
v.To participatein vaccination programmes for adults and children
w.Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice)
x.Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
y.Tosupport the clinical team with any safeguarding matters, in accordance withlocal and national policies
z.Tounderstand practice and local policies for substance abuse and addictivebehaviour, referring patients appropriately
Person Specification
Experience
- Previous experience of working within a Nursing Team in General Practice would be desirable.
- Experience of dealing with patients with learning disabilities, would be desirable.
