**Join Vital Home Care as Case Manger**
We provide support services, personal care and activities for clients in their homes and community. Whether you want to access the community through community participation or you want to get involved in activities to build skills, independence, and connections, VHC Services can assist you with daily life to develop your independence and confidence.
**Responsibilities**:
1.Conduct regular client meetings
2. Manage service delivery and ensure high quality care
3. Oversee staff induction and management
4. Assist with rostering and scheduling
5. Handle new client referrals
6. Attention to details
7. Willingness to learn new systems and following instructions.
**Requirements**:
1.Qualification in Aged care or Disability services.
2. Current police check.
3. Available monday to friday.
4. Previous experience in similar role will be preffered.
5. Proficient in computer use, professional, polite and friendly demeanor.
**Benefits**:
1. 25 hours work weekly, can be increased in future
2. Mostly work from home
3. Performance based bonus
