About the Role

This role provides support and works in partnership with people living with disability to live within the community in the way that they want to. We strongly encourage people with a lived experience of mental‑ill health to apply for this position.

We empower our customers by supporting them to build independence, identify and achieve goals, engage in activities of daily life, and encourage genuine connections within the community. Key Responsibilities

1. Collaborating with customers to develop individualised person‑centred support plans that incorporate their identified wellbeing needs, their strengths, and their goals.

2. Providing support to customers to improve their independence by assisting with skill development, transport, social activities, supporting with finances, attending appointments, cooking, medication and personal care, as well as other support as required.

3. Supporting and encouraging our customers to participate in activities of their choice, such as social, leisure, sporting, educational, employment activities etc.

4. Keeping customer records up to date on a daily basis using online systems and electronic databases.

5. Developing and maintaining professional relationships and networks with customers, families, caregivers and key stakeholders.

6. Supporting customers to navigate the sector to access any additional service they may require.

7. Seeking out innovative ways to enhance the recovery journey and overall customer experience with New Horizons.

Our customers would like you to be punctual, friendly, outgoing, non‑judgmental, flexible, guided by lived‑experience, persistent, balanced, empathetic and practical. Our customers also want you to know when to motivate, when to challenge, and when to stop

What you will bring to the role:

1. Minimum Cert III in Disability, mental health, Peer Work, or equivalent

2. Valid NSW Drivers License (Class C), with ability to drive large cars and/or vans

3. Willingness to obtain an Australian Federal Police Check, NSW WWCC and NDISWC to meet New Horizons employment criteria prior to commencing with us.

4. A strong set of values that guides your work, incorporating our organisational values of passion, integrity & respect.

5. Great computer skills with proven ability to use Microsoft Office and customer relationship management systems.

6. Desirable:

- Experience in disabilities, mental health conditions

- Advanced knowledge of services and programs under NDIS.

- Understanding of Person‑Centred Practices
