* Excellent benefits including salary packaging* Friendly and dynamic team* Permanent / Part time **(15.2 hours per week Wednesdays & Thursdays)*** Provision of Occupational Therapy assessments, management, recommendations and ongoing referrals for clients presenting to Polio Services Victoria.* Provide specialist assessments in conjunction with Physiotherapy in the area of posture and seating with Grade 3 Support.* Ensure provision is timely and to a high clinical standard, utilising accepted best practice and innovative techniques as appropriate.* Work collaboratively with the Polio Services team to provide ongoing education and advice to clients and health professionals in regard to the Occupational Therapy management of people with a history of polio.* Work with the Polio Services team in research and service development to ensure the service meets the needs of clients and is implementing accepted best practice.* The role requires supervision of student placements in conjunction with other HIP services.* Bachelor in Occupational Therapy, or other relevant degree* Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or disability and/or community based service* Understand and support the principles of HIP continuum of care for clients, particularly in relation to individualized, time-limited, goal-centered episodes of care that aim to improve health outcomes* Highly developed interpersonal skills that incorporate excellent communication, organization, planning and problem solving skills* A valid Nationally Coordinated Criminal History Check, or willing to obtain* A focus on wellbeing initiatives, with regular events and programs* Confidential, solutions-focused employee counselling* Ability to join Fitness Passport - Your pass to an extensive choice of fitness facilities* Salary Packaging – Increase your take home pay* In the heart of Fitzroy, CBD at our doorstep, close to some of Melbourne’s best cafes, public transport and lots of carparks* Discounts and Promotions always available through our Foundation* Regular opportunities for professional development to assist you to reach your career goals* Culture of continuous improvement* Provide safe and effective Occupational Therapy direct service provision in a variety of settings (including outpatient, home based, regional clinics and virtual).* and a variety of clinics including occupational therapy assessments, posture and seating and home Provide specialised assessments and interventions jointly with Physiotherapy in seating and posture and assistive technology strategies* Provide Occupational Therapy assessments and reports to enable access to assistive technology, disability and aged care funding schemes* assessments.Participate in delivery of group-based interventions such as the Pain and Fatigue Management group* Provide clinical expertise in maintaining evidence and outcomes-based service* Participate in service improvement initiatives relevant to PSV and/or the HIP occupational therapy service* Provide guidance, support, and supervision of students junior allied health staff within the HIP PSV service* Involvement with and / or encouragement of research and quality improvement relevant to polio* Assist with triaging of clients as required* Assess and manage all assigned clients as appropriate, according to HIP and St Vincent’s policies and procedures and relevant rehabilitation protocols* Maintain accurate statistics and submit by end of working week* Achieve activity targets as agreed with team leader and/or senior occupational therapist* Maintain the appropriate standard of medical record keeping and data collection as per St Vincent’s, NDIS and HIP policies* Attend Polio Services Victoria clinics – metropolitan and regional* Maintain up to date knowledge and skills in managing people with a history of polio* Establish, develop and maintain strong and positive working relationships with referral sources, participating agencies and other service providers* Liaise with community organisations and health providers involved in polio related issues and service delivery* Provide education, support and advice to referrers and treating teams regarding the management of polio clients* Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or disability and/or community based service* Demonstrated sound knowledge and application of therapy practice and principles in occupational therapy assessment, treatment and equipment prescription* Clinical experience in the area of neurological conditions* Knowledge and understanding of the NDIS and other relevant community resources and services* Understand and support the principles of HIP continuum of care for clients, particularly in relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes* Highly developed interpersonal skills that incorporate excellent communication, organization, planning and problem solving skills* Demonstrated ability to work with minimal supervision, exercise initiative and make decisions independently* Demonstrated ability to work in a multidisciplinary team* Evidence of ongoing education/professional development relevant to occupational therapy practice* A commitment to maintaining and improving occupational health and safety standards* Commitment to the values and health care philosophy of St. Vincent’s* Knowledge of and commitment to continuous quality improvement* Previous involvement in quality improvement activities* Previous involvement in research projects* Experience in service development* Membership of the Australian Association of Occupational Therapists is desirable.St Vincent’s Hospital Melbourne (SVHM) is a Catholic not-for-profit healthcare provider with a reputation for compassionate care and world-leading innovation.
