As a registered NDIS provider, we deliver mental health and disability supports designed to empower people to live independently, connect socially, and achieve their goals.

About the Role

We are looking for compassionate Community Access Workers to join our Mental Health & Disability Programs team. In this role, you will provide flexible, person-centred support for people living with disability or psychosocial challenges.

You’ll help participants with daily living, build independence, and support their participation in the community.

Key Responsibilities

- Provide outreach and in-home support tailored to participants.
- Assist with activities such as shopping, meal preparation, bills, domestic tasks, and prompting with personal care.
- Support access to community, education, employment, and recreation opportunities.
- Encourage capacity building and personal goal achievement.
- Complete accurate shift notes and administrative records.
- Build positive relationships with participants, families, colleagues, and external providers.

About You

You are empathetic, reliable and motivated to make a difference. You’ll also bring:

- Certificate III in Disability, Individual Support or equivalent (minimum).
- Experience in the disability sector.
- Current First Aid & CPR certification.
- Open QLD driver’s licence + reliable, insured vehicle.
- Blue Card & Disability Worker Screening Check (or ability to obtain).
- National Police Check.

Desirable: Experience with strength-based and capacity-building approaches.
