The Role

The social worker will provide high quality care and clinical expertise and specialised knowledge when planning, coordinating and implementing care for the Palliative Care patient cohort which consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards. The social worker will have responsibility for providing clinical care and care coordination to people across SESLHD who have end of life and palliative care needs and are admitted to St George Hospital. This position is suitable for clinicians interested in palliative care. The position will assist with triage and assessment of inpatients to the appropriate community or outpatient social support services. The social work position is part of the Palliative Care MDT and will provide comprehensive health assessments, therapy, education and support to patients and their families for patients admitted to St George Hospital. The successful candidate will be supported by the Palliative Care Teams at St George Hospital as well as by the social work team. In addition to the above, there will be opportunities to lead education and research activities and undertake strategic planning and capacity building activities for this service to identify unmet palliative care and end‑of‑life needs, promoting early referrals to specialist palliative care services, and supporting communication between the ward‑based clinician and the palliative care multidisciplinary team (MDT).

Join Our Compassionate and Skilled Social Work Team at St George Hospital

We are seeking a dynamic and experienced Level 3 Social Worker to join our dedicated team in a permanent full‑time role within the inpatient palliative care service at St George Hospital. This is a unique opportunity to work in a specialist clinical area, supporting patients with palliative cancer and non‑cancer diagnoses, their families, and carers. The successful candidate will play a pivotal role in delivering holistic, person‑centred care as part of a multidisciplinary team (MDT) in palliative care.

Key Responsibilities

- Previous experience working in Palliative Care is desirable.

- Providing advanced psychosocial assessment and intervention for patients and families facing life‑limiting illness.

- Supporting complex discharge planning, grief and bereavement care, and end‑of‑life decision‑making.

- Collaborating closely with medical, nursing, allied health, and pastoral care teams to ensure integrated care delivery.

- Offering clinical supervision and mentoring to junior social work staff and students.

Selection Criteria

- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for membership of the AASW

- Comprehensive post graduate clinical experience in the provision of social work intervention with clients with end of life or palliative care needs

- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels

- Ability to provide in‑depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff

- Ability to work as part of a multi‑disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a complex or critical nature

- Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff

- Demonstrated commitment to ongoing clinical and professional learning

- Participation in the After Hours Crisis On Call Social Work Service
