What you will bring to the role

- Has undertaken or willing to undertake a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care where the role is to be assigned (including undergoing recognition of prior learning processes against current qualifications).

- In this role Aboriginality is a genuine occupational qualification and is authorised by section 14(d) of the Anti-Discrimination Act 1997. Applicants for this position will be required to cite their Aboriginality. - Demonstrated experience with or willingness to learn the relevant policy and procedures within the Local Health District and NSW Health that guide the Care services in Primary Care and Community Health

- Demonstrated ability to provide cultural brokerage with individuals, carers, families and communities; cultural guidance for the other members of the team; and, facilitate respectful and culturally safe access for individuals, carers, families and communities

- Demonstrated experience working with and ability to provide social support for, people with chronic and complex care needs, in collaboration with the multidisciplinary teams

- Demonstrated understanding of the health and psychosocial issues involved with people who have chronic conditions, including the ability to communicate well with Aboriginal people, families and communities, and network with other services and professionals

- Demonstrated good written, oral communication and computer skills

- Current NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD
