About The Role
We’re seeking a compassionate and organised SAH Team Lead (previously HCP) to lead our care operations. In this leadership role you will oversee and mentor the SAH Care Partners team, ensuring the seamless delivery of Support at Home services (previously Home Care Package services) that align with each client’s individual needs, preferences, and goals.
As an Team Lead, you will oversee the coordination of care services for older adults living in the community while continuing to perform key client-facing tasks. You’ll collaborate closely with clients, families, health professionals and service providers to maintain current care plans, manage risks, coordinate services, and ensure high quality service delivery.
Key responsibilities include:
- Lead, support, and mentor the SAH (formerly HCP) Care Partners team
- Ensure care and services meet clients’ needs, goals, and preferences
- Conduct regular client reviews and home visits
- Identify and manage risks to client safety, maintaining accurate documentation
- Respond to incidents and feedback in a timely and appropriate manner
- Coordinate services with internal and external providers
- Act as a key contact for clients, families, GPs, and allied health professionals
- Organise family conferences and multidisciplinary review meetings
- Keep clients informed of changes to agreements, care plans, and budgets
- Participate in on-call duties every sixth week (RDO provided in lieu)
- Perform other duties as directed by management
About You
You’re a strong communicator with a genuine commitment to delivering high-quality care. You thrive in a collaborative environment and are confident managing competing priorities. Your attention to detail and organisational skills ensure clients receive the support they need, when they need it.
You’ll also bring:
- Case management experience, ideally in aged care or community care
- A sound understanding of Home Care Packages (and SAH) and sector compliance requirements
- Excellent communication and relationship-building skills
- Strong time management and organisational abilities
- Proficient computer skills
- Clinical registration and experience (e.g. EN, RN) (desirable)
- Demonstrated leadership or team supervision experience
- A collaborative mindset with a commitment to fostering a positive team culture
