We are looking for a passionate leader who is looking to grow and further develop their career with an organisation dedicated to giving back to the community.

As a Team Leader Coordinated Care, you will report to the Prevention and Partnerships Manager and work with a team made up of three streams; Pharmacotherapy Area-Based Networks (PABN), Financial Counselling and Supported Residential Services Outreach Assistance Program . You will support the team in the planning, implementation, evaluation and reporting of these programs, as well as day to day operations and administration. This role will involve strategic work, such as development of new programs and seeking additional funding. There will also be a stakeholder management component to this role.

To be successful, you will require;

- Bachelor's degree of Nursing, Pharmacy or a related clinical field
- Victorian Drivers Licence
- A current Employee Working with Children Check
