Overview

Clinical Care Coordinator – Adelaide CBD, Full Time

Responsibilities

- Progress with purpose at one of Australia’s largest health care providers
- Get ahead with salary packaging, benefits and professional development in a supportive team environment.
- Based in Adelaide CDB, coordinate clinical services to support clients in their own home.
- Be the primary point of contact in supporting clients and staff to ensure services are delivered efficiently and provide clients with the best possible outcome.
- Daily coordination of care for new and existing clients of the SA Health Community Care Program.
- Acceptance of referrals including the Metropolitan Referral Unit (MRU) and ensuring all client information is available to commence service delivery within required service delivery timeframes.
- Identify clients in the clinical programs that are eligible for other Government funding streams and liaise with the referrer, MRU or healthcare professional for transfer of care.

Qualifications

- Bachelor of Nursing
- Experience in Acute Nursing
- Community nursing experience
- Full Driver’s Licence
- Previous experience managing service contracts
- Previous experience managing staff
- Strong problem solving and time management skills
- Effective communication skills
- Computer skills
- Ability to work over a 7 day roster
- Logistical experience (desirable)
